First Appointment Checklist:

  • Insurance Cards (Primary and Secondary)
  • Insurance Co-Payment
  • Referral Form from Referring Physician
  • Copy of your medical chart from your primary care physician, as it pertains to your cardiovascular health
  • Medication List with Dosages (Actual Containers Preferred)
  • List of Drug Allergies

During your first visit, your physician will assess your condition and discuss your symptoms and medical history with you. The doctor may then order a variety of tests to further aid in diagnosing your particular condition.

Our Mission

Our nine cardiologists and three mid-level providers specialize in the treatment of heart disease, with emphasis on reducing cardiac risk factors, the prevention of heart disease, and the promotion of overall good health.

We work with patients to prevent disease through lifestyle changes that impact a person's risk factors, and coordinate rehabilitation after a heart attack or development of heart disease.
